Please bring the following documentation to Flat Rock ES when registering a student.
According to Georgia law, each student entering a Georgia Public School must have the following:
• Updated Proof of Residence or Affidavit (current gas, electric bill)
• Birth certificate (State Certified)
• Immunization (Form 3231)
• Hepatitis B (#doses) for K students born after January 2, 1996
• Varicella Vaccine/Chicken Pox is required for students entering the 6th grade (2023-2024)
• Proof of Immunization for two doses of measles for all students/ all grades new to Georgia
for the first time
• Hearing/Vision/Dental (Form 3300)
• Social Security Number
• Guardianship Papers (if applicable)
Print and complete the Registration Checklist to ensure you have all the necessary documents for your registration process.

Withdrawal Process
Parents may come to the school to fill out the withdrawal Form please bring ID.
Parents who are not able to come to the school please email [email protected] with a copy of your Id requesting Withdrawal Form.
